We used transcutaneous vagus nerve stimulation (t-VNS) to treat patients with intractable epilepsy. Our aim was to assess the therapeutic efficacy and safety of this technique. Nine patients with refractory epilepsy (5 females, 4 males) underwent t-VNS treatment. Demographic data, seizure semiology, antiepileptic drugs (AEDs) and scores in the Epilepsy Surgery Inventory (ESI) were recorded. Device settings were as follows: stimulation frequency, 20 Hz; pulse width, 30-500 micros; and stimulus intensity, a current of 15 mA. Safety parameters were daily patient-reported discomfort and distress during the treatment period. Clinical response was defined as >50% reduction in seizure frequency, seizure reduction greater than 75%, seizure-free status for a period of at least 3 months and a minimal 3-month response. Patient tolerability was high (median overall discomfort: 0, range 0-2; median overall distress: 0, range 0-2) and no serious adverse events were recorded. In 2 patients, complete seizure control and reduction of seizure frequency to less than 20% were achieved. In 6 patients, a >50% reduction of seizure frequency was achieved, and in 3 patients, a >75%
